Platform Information Submission

Verify and confirm whether the drug with ADR has intact packaging and tight sealing, and confirm whether it is a product of our company and within the validity period
Time and location of adverse reaction occurrence, diagnosis and treatment process, health status, reason for medication, medical history, course of disease, medication start and end time, patient's dosage, doctor's prescription. Compare with the instruction manual to check if there is any medication against the instruction manual (e.g., combined medication, compatibility and dosage of combined medication). Check if there is a family history of previous adverse drug reactions/events, and whether the patient has a history of allergies, smoking, drinking, pregnancy, liver disease, kidney disease, etc. (A copy of the prescription can be attached)
Description and Treatment: Describe the main manifestations of adverse reactions and the treatment of the patient (including when the adverse reaction occurred, when the drug was discontinued, what measures were taken, when the adverse reaction was cured or improved, etc.)
User Information: Basic information and qualifications of the investigated user's unit or hospital (clinic) to check if they meet the requirements. Number of people using the same batch of the drug in the hospital (clinic) and the number of people with adverse reactions, and inventory of the same batch in the using unit
Attending Physician: Qualifications of the attending physician and relevant personnel (such as nurses) (education background, years of medical practice, history of using the drug) to check if they meet the relevant requirements
Warehouse Conditions: Check if they meet the storage conditions of the drug (e.g., temperature and relative humidity of the warehouse)
